Information you send
The contact form asks for your first and last name, email address, and message. You may also select a session you are interested in. Your submission is saved in a private inquiry inbox available to the site owner so Dallin can review and respond to it. Use the form for general service and scheduling questions. Please do not include medical records, detailed health histories, payment-card details, or other sensitive information.
How inquiries are used
Dallin uses inquiry details to respond, discuss a possible appointment, and handle the request you made. The website does not publish your message. Submitting an inquiry does not subscribe you to a marketing list.
Online scheduling
The booking page connects to Acuity Scheduling, a third-party scheduling service. If you load or open the scheduler, Acuity receives information needed to provide its service, and information you enter there is handled through Acuity. Its privacy and cookie practices are described in the Squarespace privacy policy. Review those terms before entering information.
Optional analytics and preferences
If Google Analytics is configured for this website, it is loaded only after you accept optional analytics. You can decline optional analytics and continue using the website. Analytics may record page views and general interactions such as selecting a contact or booking link; contact-form names, email addresses, and message text are not sent as analytics event data. You can change your choice using the website's cookie preferences control.
A preference stored in your browser helps remember your choice. Third-party services such as the scheduler may use their own cookies or storage when you choose to load them. Your website analytics choice does not change a third party's separate settings.
Service providers and access
The site uses hosting and database services to operate the website and save inquiries. Acuity provides scheduling, and Google provides optional analytics when enabled and accepted. Information sent by email is handled by the email service used for the practice. Access to the inquiry inbox is restricted to the owner account; no online service can promise absolute security.
The form uses temporary technical records to limit spam, including a hashed network identifier and an expiring submission token. These records help prevent repeated or automated submissions. They are separate from the message you choose to send.
Questions and deletion requests

Provide enough information to identify the inquiry without sending additional sensitive details. Requests involving information entered into Acuity may also require action in that service. This notice may be updated when the website's features or practices change.
